Federal/State Tax Forms
Each employee will be required to have an updated federal and state withholding form on file. The W-4 governs federal tax deductions and the A-4 governs state tax withholdings. It is a requirement that all employees update these forms in January of each year. Forms are available in the Payroll Office of the Human Resources Department.
